Signing up
In order to participate in NSAP, you must first SIGN UP. A sign up form is available here.
Sign up is the first step, prior to beginning the self-assessment process.
 

 
NSAP - the first cycle
Because you are participating in your first cycle of NSAP - the NSAP National Project Team is working in collaboration with all Australian state and territory health departments to ensure that you recieve the best support possible.
For the first cycle of NSAP, there will be a number of differences to future self-assessment cycles.
  • States and territories are being scheduled by the NSAP team in collaboration with the relevant health department, to do NSAP together as a group.
    This ensures that the roll-out of NSAP is integrated with State and Territory priorities and has the full support of jurisdictional health departments.
  • The NSAP team will contact all Specialist Palliative Care Services in each state or territory, in the leadup to their scheduled start date.
  • An NSAP workshop will be held in each state or territory, to provide services which information and training in NSAP. All services within the state or territory will be invited to attend.
  • Should you wish to complete and submit the below sign up form prior to the NSAP team contacting your service, the NSAP team will happily accept this. 
    Please note - your sign up form will not be processed until your state or territory NSAP process begins
Should you prefer to wait until the NSAP team contacts all the services in your state or territory, prior to sign up but would like to confirm that we have the correct contact details for your service, please email nsap@palliativecare.org.au with your service name and details.
 

 
What happens after you sign up?
Once you have signed up, the NSAP team will provide you with further information and resources to assist services prepare for the self-assessment process.
Your service will recieve:
  • Access to secure sections of the website, which provides information and resources developed specifically to assist in preparing for the self-assessment including;
    • What is evidence, how to generate evidence or identify existing evidence
    • What is a multi-disciplinary team (MDT), who should be involved and more
    • Strategies for success
    • Experiences and advice from others
During the preparation period, your service should prepare for self-assessment.
  • There is NO maximum time that a service can take to prepare, prior to beginning self-assessment, but should your service not have begun self-assessment after a period of time, a member of the NSAP team may call to check on your progress.
  • Gathering evidence can be a time-consuming - taking the time to identify or undertake data generating activities prior to undertaking the self-assessment can make the process easier.
  • Identify who should be in the multi-disciplinary team
  • Begin to inform other staff in your service of NSAP and what it is - the brochure you recieve will assist with this
    • Support from your colleagues and management can improve your ability to complete the self-assessment and once the MDT have completed the self-assessment and developed an acion plan, support long term change.

Once recieved, the NSAP team will process your service's form and send an introductory pack.

Specialist Palliative Care Service Levels

When services complete the sign up form, they are asked to identify their service level. There are three specialist palliative care service levels.

In 2005, Palliative Care Australia developed A Guide to Palliative Care Service Development: A population based approach which outlines the capabilities and typical resource profiles of primary care and specialist palliative care level 1-3 services.
